Try the New NHS App

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.  

You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.
If you already use www.patientaccess.com you can continue to use it. You can use the NHS App as well. For more information go to www.nhs.uk/nhsapp
 
Out of area registration scheme
The out of area registration scheme launched by the government is voluntary and this practice has chosen to participate in this scheme. This means we are  able to register patients living outside our catchment area, however this is discretionary and may not be appropriate for all patients. Please speak to one of our receptionists for more information

HOW TO CONTACT YOUR HEALTH VISITOR

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.

Dear Parents

Please book the following appointments with your GP

At 6-8 weeks - BABY ASSESSMENT

At 6-8 weeks- MOTHERS POSTNATAL REVIEW

At 8 weeks   - BABYS FIRST IMMUNISATION (1ST)

At 12 weeks - BABYS SECOND IMMUNISATION (2ND)

At 16 weeks - BABYS THIRD IMMUNISATION (3RD)

At 12-13 months- BABYS FOURTH IMMUNISATION (4TH)

At 3YEARS 4 MONTHS - PRE SCHOOL BOOSTER

Your Health Visitor will discuss withy you  your babys BCG vaccination and issue you with an appointment. It is very important that you attend this immunisation as your baby is at risk of TB if not immunised

Please remember to complete page 25 of your red book BEFORE attending your 6-8 week review
